The Lakes Home Health Services, Inc. is an independently owned and operated provider of home health care services, such as skilled nursing and rehabilitative therapies, to homebound patients. We serve in the areas of Los Angeles and Kern , California and its surrounding communities.

How It Works
- Ask for an assessment of current living skills to determine whether you or your loved one is eligible for home health care.
- Submit a doctor’s recommendation (with supporting documents) for home health care.
- Once eligibility has been affirmed, our health professionals will work with your attending physician to create a customized care plan tailored to your specific needs.
- We will collaborate with you, your family members, and others involved in the care process to put the personalized plan of care into action.
- We will monitor the implementation of our services to apply apt changes and modifications when/if necessary.
- Feedback will be provided, and the patient’s condition and care plan will be reassessed.
